If you're approaching 65 on the Gulf Coast — whether you live full-time in Gulf Shores, are a Pensacola retiree, or split the year between Foley and Florida — Medicare is about to become the biggest healthcare decision you'll make for the next 20+ years. The wrong choice can cost you hundreds of dollars a month and leave you fighting denied claims. The right one saves you money and keeps every one of your doctors in network.
Here's everything you actually need to know, written in plain English, with the specific Gulf Coast wrinkles that most national guides leave out.

The Medicare basics: Parts A, B, C, D explained
Medicare has four parts, and yes, it's confusing on purpose. Here's the cleanest summary you'll find:
- Part A — Hospital insurance. Covers inpatient hospital stays, skilled nursing facility care (limited), hospice, and some home health. Most people pay nothing for Part A because they paid into it via payroll taxes for 10+ years.
- Part B — Medical insurance. Covers doctor visits, outpatient care, preventive services, ambulance, durable medical equipment, lab tests. Has a monthly premium ($185/month in 2026 for most enrollees, higher if income exceeds $103k single / $206k married). Deductible plus 20% coinsurance on most services.
- Part C — Medicare Advantage. A private plan that REPLACES Original Medicare (Parts A + B). Often includes Part D drug coverage, dental, vision, hearing.
- Part D — Prescription drug coverage. Either bundled into Medicare Advantage or purchased separately to pair with Original Medicare.
The most important sentence in this post: You will choose between Original Medicare + Supplement + Part D OR Medicare Advantage. These are two completely different paths and you can't easily switch between them later.
Original Medicare vs. Medicare Advantage — the real difference
This is the choice that matters.
Original Medicare + Medicare Supplement (Medigap) + Part D drug plan
Parts A and B from federal government + Medicare Supplement (typically Plan G) to fill the 20% Part B gap + standalone Part D for prescriptions.
Pros:
- See ANY doctor/hospital in the U.S. that accepts Medicare. No network restrictions. Huge for snowbirds and specialty care (Mayo, Cleveland Clinic, MD Anderson).
- Predictable cost — Plan G covers everything except a small annual Part B deductible ($257 in 2026).
- No referrals or prior auth for most services.
Cons:
- Higher monthly premium. Plan G in AL/FL for healthy 65-year-old: $140–$220/month, plus Part B $185, plus Part D $25–$60 = $350–$465/month.
- Dental, vision, hearing NOT included.
Medicare Advantage (Part C)
Private plan replaces Original Medicare.
Pros:
- Many Gulf Coast plans have $0 monthly premium beyond Part B.
- Most include dental, vision, hearing, gym membership. Some have OTC cards ($50–$150/month).
- One card for everything.
Cons:
- Network restrictions. Limited doctor list — much smaller than "any doctor in the U.S."
- Prior authorizations and referrals for specialists.
- Out-of-pocket max $5,000–$9,000/year.
- Plan benefits/networks change every year.
The honest truth
There's no universally "better" option. Medicare Advantage is usually better if you're healthy, see 1–2 doctors, want low monthly cost, don't travel much. Original Medicare + Supplement is usually better if you see specialists, travel/split states, want zero network worry, and can afford the higher premium.
Gulf Coast clients: roughly 60/40 toward Advantage, with snowbirds and frequent travelers leaning Supplement.

Call (251) 279-7007The 3 enrollment windows you cannot miss
1. Initial Enrollment Period (IEP) — when you turn 65
7-month window: 3 months before your birthday month, your birthday month, 3 months after. If your birthday is August 12, IEP runs May 1 – November 30.
Critical: Skip Part B enrollment without qualifying employer coverage → 10% Part B premium penalty per 12 months delayed, for life.
2. Annual Enrollment Period (AEP) — October 15 to December 7 every year
Anyone with Medicare can:
- Switch Original ↔ Advantage
- Switch Advantage plans
- Add/drop/change Part D
Changes take effect January 1.
This is when most Gulf Coast clients call us. Plans, premiums, and drug formularies change yearly — even if you love your current plan, compare alternatives during AEP.
3. Medicare Advantage Open Enrollment — January 1 to March 31
One-time chance to switch to a different Advantage plan or drop back to Original Medicare. Can't change Supplement during this period.
Special Enrollment Periods (SEPs) also exist for life events: moving, losing employer coverage, etc.
What Medicare actually costs in 2026 (real numbers)
Healthy 65-year-old on Gulf Coast:
| Component | Original path | Advantage path |
|---|---|---|
| Part B premium | $185/mo | $185/mo |
| Medicare Supplement (Plan G) | ~$165/mo | $0 |
| Part D | ~$40/mo | Usually included |
| MA premium | n/a | $0–$60/mo |
| Total | ~$390/mo | ~$185–$245/mo |
| Deductibles | $257 (Part B) | Varies $0–$500 |
| Out-of-pocket max | Very low | $5k–$9k/yr |
| Dental/vision/hearing | Not included | Usually included |
Higher-income earners ($103k+ single / $206k+ married) pay IRMAA on Part B + D premiums.
Gulf Coast specifics most national guides skip
Doctor networks in Baldwin / Mobile / Escambia counties
Major Advantage networks here: UnitedHealthcare, Humana, Aetna, Wellcare, Blue Cross Blue Shield of Alabama. Biggest gaps at Infirmary Health, Thomas Hospital, North Baldwin Infirmary, Sacred Heart Pensacola.
Practical tip: Before enrolling in any Advantage plan, hand the plan name to your doctor's office and ask, "Do you accept this?" Don't trust the plan's online directory alone.
Snowbirds and dual-state living
Huge on the Gulf Coast. If you spend 3+ months in another state:
- Most Advantage plans cover urgent care nationwide but routine care only in home region.
- Original Medicare + Supplement has no such restriction — any U.S. doctor accepting Medicare accepts your supplement.
- AL/FL border residents specifically should ask about cross-state coverage.
The 5 most common Medicare mistakes new enrollees make
- Auto-renewing the same Advantage plan year after year without comparing. We've seen clients pay $1,200+/year extra because their plan's preferred pharmacy switched.
- Not enrolling in Part B because "still working" without checking employer coverage. Employer with <20 employees usually does NOT count as creditable coverage.
- Skipping Part D because no prescriptions. Permanent 1%/month late enrollment penalty for life.
- Choosing Supplement on price alone without checking renewability. Plan G benefits are standardized; rate increases vary by carrier.
- Not telling your agent about every doctor and medication. We can run your specific list against every plan in your zip in 15 minutes.
